As Assistant Home Care Coordinator you will be an active member of the St George Home Care Service, supported by a highly motivated, consumer focused Home Care Coordination Team. You will liaise and interact with other like-minded community-based service providers and health professionals.

HP3 or HP4 Dietitian required for locum with remote health service based in South West Queensland. You will join a small allied health team and cover a varied caseload including:

• Inpatients
• Outpatients
• Outreach
• Full time hours, Monday – Friday
• Late September/early October for two months + possible extension
• Preference if you are already credentialed with QLD Health
• Please only respond if you have current APD status and you are available to start in the next 2-3 weeks

The Community Connector provides specialist and intensive assistance to help older people within the target population to understand and access aged care and connect with other relevant supports in the community. This involves utilising an assertive outreach approach to engage and build rapport with potential clients as well as local intermediaries, stakeholders and networks.
